Please use one of the forms below to start the process of requesting your equity earned through qualified purchases at ALCIVIA or a prior merged cooperative.
Requests can be made based on age payout, estates or death, or dissolution of the business. They are paid out quarterly or annually based on the board of directors voting timeline.

The Patron Note Program allows members additional opportunities to invest in their cooperative. The program, established in April 2019, includes three interest-bearing certificates.
